Patient Information

Collecting relevant information about a patient's medical history and current condition is essential to providing patients with the appropriate treatment. We use this information to get a comprehensive picture of each patient's health and use that as a guide in building their customized treatment plan. To make this step a little easier, we have provided a way to do that online.

This patient information will allow our team to develop a personalized treatment plan for your unique needs and preferences. We will need to review the patient’s medical history, current condition, any existing medications, allergies, and more. You will want to have all your insurance information ready to bring with you, along with a list of your prescriptions and current medical conditions. If you receive treatment after a work injury, be sure to bring your worker’s compensation information and documents. Be sure to include any over-the-counter medication and herbal supplements you take as well.

    New Patients

    After booking your first appointment, you will receive an email containing the Patient Intake Form. Please complete and submit this form 24 hours before your scheduled appointment. The information contained in this form is a critical component of creating a treatment plan based on your needs.

    Please remember to bring your insurance ID card along with your drivers license on the day of your scheduled appointment.
